1) What are your office hours?
I schedule appointments Tuesday – Friday, daytime and evenings. I am also available for appointments on Saturday. Phone sessions are also available, but are not typically covered by insurance.
2) How do I schedule an appointment?

3) How long is a sesion?
A session is approximately 50 minutes. If you arrive early for your appointment please be seated in the waiting area.
4) What should I bring to my first appointment?
Please fill out the new client forms and bring them with you to your first appointment. If you plan on using Blue Cross / Blue Shield you will also need to provide your insurance card.
5) Do you accept insurance?
I am an in-network provider for Blue Cross / Blue Shield PPO. If you have another provider I can supply a bill for you to submit to your insurance company for possible reimbursement.
Please contact Blue Cross / Blue Shield prior to the first appointment to inquire about any deductibles, co-insurance and co-pays that may apply to your session. Also, you may need prior authorization depending on your plan.
6) What forms of payment do you accept?
I accept cash, checks, Visa and MasterCard. Payment is requested at the time of service. Click for Credit Card Authorization Form
7) What is your cancellation policy?
Therapy appointments are made in advance and this reserves my time for you. If you need to cancel and have an evening, (4:00 PM or later) or weekend appointment, I am asking for a 48 hour notice due to my limited evening and weekend hours. This is so I may schedule another client in that slot. If you have an appointment during the day I require a 24 hour notice.
All cancellations must be made by calling my office at (773) 665-2395. I do not accept cancellations through e-mail. If you do not give me proper notice to cancel or reschedule during the same week you will be charged the full fee for the missed appointment. The exception to this policy is a medical emergency or a natural disaster.
Please note that Blue Cross / Blue Shield does not cover for missed appointments.
